Evidence shows that interventions which can prevent or delay disablement and enhance wellbeing in older people include: increasing physical activity; case review; access to assistive devices and home modifications; and access to opportunities for social interaction. It is less clear, however, how to best support older people to access and use such resources, particularly in areas of socio-economic deprivation.
Recent work by this group has identified general practice as a potential ’hub’ in which to embed support for wellbeing. This project aims to develop a system by which general practices in deprived areas can identify older people in need and help them to access such resources and/or activities. The project will use an innovative, iterative and participatory approach to system re-design, involving summary of research evidence, older people, community-based workers and general practice staff. We will ’road test’ the prototype system in three deprived general practices and assess the system’s potential for wider implementation.
