Joining the study involves a few simple steps:

  1. Ask your parent/guardian to sign the Parent Consent Form online (see links below),
  2. You and your parent/guardian will receive a unique Participant ID and survey link via email, and finally
  3. You can then sign the Consent Form and complete the survey online!
